New Patients

Before we can schedule an appointment we must have the following:
a.) All completed forms and questionaires
b.) All requested laboratory results

This information is reviewed prior to your first visit.

You must bring all requested X-Ray films of problem areas at the time of your first visit.

We are a fee-for-service office and only accept cash or checks.

We will provide an encounter form at each visit for you to file with your insurance.

We do not accept medicaid, medicare or private insurance.
